In preparation for  your appointment to help complete a benefit form, try to have with you the following documents:

  • a copy of a repeat prescription list if you are on regular medication
  • any medical evidence to support your claim such as reports from specialists, but which are no more than a year old
  • a list of all the professionals you see regarding your disability, along with their contact details and dates of when you last saw them.
  • finally, don’t forget the form itself and the envelope it came with.
